The overall goal of the Community Impact Grant program is to promote health equity in Wisconsin by supporting community and academic partnership initiatives that address the social determinants of health and the causes of health inequalities and are informed and implemented by those most affected. As a Partner Program member, you can select the Self-Directed Supports (SDS) option to manage some or all of your long-term care services. Choosing SDS means having more impact on how and from whom you receive services. Take care of all your needs. Our partner program is designed to provide fully integrated care, including medical, hospital, home and personal care services. The Partnership Program helps frail seniors and adults with disabilities with higher care needs coordinate their medical, health and long-term care services. Partnership members have access to medical care and long-term care services without having to worry about insurance bills, co-payments, deductibles or insurance gaps. Medicare partnership members also receive prescription and some over-the-counter medications from Community Care, and have no co-payments or deductibles for the drugs. Members of the partnership who do not have Medicare receive prescription drugs and some over-the-counter medications from Wisconsin Medicaid and may have co-paid for the drugs. These members should contact Wisconsin Medicaid Member Services at 1-800-362-3002 for more information. This year`s funding cycle also considers the disproportionate impact of the COVID-19 pandemic on some Wisconsin families and communities. These include ethnic groups and ethnic minorities who were at higher risk of contracting and dying from COVID-19, as well as families who have been victims of deaths from COVID-19, young populations with social and emotional health problems at high rates, and rural communities that may have higher rates of smoking, high blood pressure and obesity, as well as less access to health care and less access to health insurance. Each scholarship is valid for a maximum of $1 million over a five-year period. The Wisconsin Partnership Program`s Advisory and Oversight Committee expects to receive up to three awards. Community Care`s Family Care Partnership Program (HMO SNP) („Partnership“) for frail seniors and adults with physical, mental or developmental disabilities provides comprehensive medical and long-term care. Community Care Partnership also offers prescription drugs to Medicare members. Partnership members who do not have Medicare will receive medication from Wisconsin Medicaid and should contact Wisconsin Medicaid Member Services at 1-800-362-3002 for more information. The goal is to help you live as independently as possible in the community. You play an active role in your plan. Partnership members, their families and authorized representatives are encouraged to play an active role in the development of the care plan.
